Employee Out-of-Pocket Costs

Depending on your health plan, you may have a co-payment or deductible associated with the services you seek. It is important to understand the guidelines of your particular plan prior to the birth. To find out what's covered under each medical plan option, check out the plan summary on each health plan by visiting the Staff Benefits Medical Coverage page.

Pre-certification for birth

For all Purdue health plans, if you are in-network the hospital/doctor is responsible for any pre-certification process. If you are out-of-network, you are responsible for any pre-certification process.

Adding a Child to Your Health Plan

You have 30 days following the birth, adoption, or placement to add the child to your medical, dental, vision, and life insurance plans.

It is your responsibility to submit new benefit elections when you have a change in family status. To make the change, sign on to Benefitfocus, Purdue's online benefits system. Select “Life Event Enrollment Changes” from the right-hand column. Once this has been processed and other necessary documentation have been received, the family status change will cover medical expenses as per plan guidelines back to the event date.

Tax Considerations

You may want to consider changing the exemptions on your tax form (W4 form). Sign into the OnePurdue Portal, and 1) click on the Employee Self Service tab, 2) click on the Payment icon, 3) and click on W-4/WH-4 Form, and 4) continue with the instructions on the form.
