A Note about Orthodontia Plan and ages: Orthodontic services (braces) are only covered under Option 1 for dependent children ages 8-18. Age limitation: A child must be banded between ages of 8-19 in order to receive coverage. If children are dependents until age 19, they can continue to receive coverage, but they must have been banded before age 19. Orthodontic payments: Because orthodontic treatment normally occurs over a long period of time, benefit payments are made over the course of treatment. You must have continuous eligibility under the plan in order to receive ongoing orthodontic benefit payments. This means you cannot change or drop plans while the plan is still making payments. If you do, you’ll lose whatever amount was not paid yet. Learn more about your dental coverage on the benefits website or anthem.com.
Mid-treatment with Orthodontia? The increase to the orthodontia benefit impacts those with dependents mid-treatment. After Jan. 1, 2021, you’ll need to file a new claim with the original banding date, total treatment fee and treatment length. The allowed amount is determined after checking if the provider is in-network. See an example of how this works. VISION What's New In 2021, Purdue’s vision plan is a separate election.
