Exams and Committee Meetings

Forms and Rubrics

Current students are responsible for downloading, completing, and submitting the appropriate forms below on time to meet the graduate school deadlines by following the instructions below.

Completed forms should submitted to the ESE Graduate Program Coordinator via email, campus mail, or in person:

 Submitted forms will be reviewed by the ESE Program Head, Dr. Linda Lee.

Committee Meetings

  • Complete the ESE Graduate Student Committee Meeting Report Form for each committee meeting you hold.  Committee meetings should be held at least once a year.
  • Submit completed form to ESE Graduate Program Specialist via email, campus mail, or in person.
  • Check with your degree granting department to see if they have department specific forms to complete for committee meetings.

MS Thesis - Final Exam/Defense

  • At least 3 weeks before your exam date, submit the electronic Form 8 (Request for Appointment of Examining Committee).  Approved Form 8s must be received by the Graduate School at least 2 weeks prior to the exam date. Follow Form 8 Instructions for a step by step guide to submit your Form 8
  • A few days before your exam, send your committee members the ESE MS Thesis Defense Rubric and Rubric Cover Page.
  • Check with your degree granting department to see if they have department specific forms to complete for MS Final Exams/Defenses.
  • After your exam, your committee members should:
    • Record the results of your exam on an electronic Form 7.  This records your results with the Graduate School.
    • Complete the ESE MS Thesis Defense Rubricand Rubric Cover Page and return them to the ESE Graduate Program Specialist via email, campus mail, or in person.
  • After your exam, you should follow the Thesis Deposit Process and submit your Electronic Thesis Acceptance Form.

MS Non-Thesis

  • During the semester that you are registered to graduate, an electronic Form 7 must be completed.
  • The ESE Graduate Program Specialist will generate the Form 7 and the committee members will record their responses.  The committee or department may require a meeting with or without the student to discuss their work and determine whether they recommend the student receive their degree.
  • Check with your degree granting department to see if they have department specific forms to complete for MS Non-Thesis degrees.

PhD Preliminary Exams

  • At least 3 weeks before your exam date, submit the electronic Form 8 (Request for Appointment of Examining Committee).  Approved Form 8s must be received by the Graduate School at least 2 weeks prior to the exam date. Follow Form 8 Instructions for a step by step guide to submit your Form 8
  • A few days before your exam, send your committee members the ESE PhD Preliminary Exam/Proposal Rubric and Rubric Cover Page.
  • Check with your degree granting department to see if they have department specific forms to complete for PhD Preliminary Exams.
  • After your exam, your committee members should:

PhD Final Exam/Defense
