Self Identification Compliance Form Updated for Veterans’ Status - 02/12/09

Human Resource Services has updated the Self-Identification Compliance Form (SICF). The revision complies with new veteran status categories, which the federal government requires the University to collect and report on for its workforce.

The revised form includes a new check block, “Veteran -- Not Included in Above Categories,” for veterans who do not currently fall into one of the form’s three listed and defined categories: “Disabled Veteran,” “Other Protected Veteran” and “Armed Forces Services Medal Veteran.”

The new category is useful, for example, for Vietnam-era veterans, who may no longer be protected by one of the categories allowed. Such veterans now can inform the University that they are veterans, even though they don’t fit into any of the specific categories listed.

Please remind veterans completing the form to provide their PUID and name on the top of the form, and then to complete applicable categories noting that more than one category may apply based on the individual situation.

It also is important that veterans provide their most recent discharge date.

The Office of Institutional Equity, formerly the Affirmative Action Office, will be sending out information to all employees to resurvey the University workforce before academic year faculty and staff depart at the end of this semester. This effort will be communicated campuswide in Purdue Today and Inside Purdue. Veterans can update their information by completing the either SICF or the information in Employee Self Service (ESS).

ESS is accessible by logging onto the OnePurdue portal available on the home page of the University’s Web site. It will be updated with the new  information by March 1.
