Process for Submitting Forgery Affidavits Has Changed - 06/2/09

The Office of Treasury Operations (OTO) has updated the process for handling Forgery Affidavits, and it is available on the Business @ Purdue Web site.
The process explains the steps the department, school or campus must take when a lost or stolen University check is cashed. The revised process instructs staff to obtain a Forgery Affidavit from the OTO, instead of the Accounting Department.

The Forgery Affidavits process can be accessed on Business @ Purdue by selecting “Finance” and then “Cash and Banking.”
