Donor Scholarship Agreement

• Establishing a scholarship in the University Honors Program at Purdue University can be done by completing a Scholarship Agreement Form. We would like to encourage any prospective donor to call the Office of Development at 765-494-9057 or 800-893-4108 for guidance in completing the form.

• The completed document should be sent to the address at the bottom of the form along with a check or money order made payable to the Purdue Research Foundation. Gifts made by credit card will need to be made through the Office of Development.

• The Scholarship Agreement may be modified at a future date, if desired, but only after an award cycle has been completed in which the current criteria was advertised.

• A General Scholarship Fund for the University Honors Program was established to receive gifts of any amount and the funds used to support additional scholarships. No agreement is requred for this type of gift.

• The minimum scholarship award is $500.
