Insurance Plan Overview

*Introduction:
The Purdue International Student Insurance Plan is a comprehensive program designed for the International student population at Purdue University. Student Resources, underwritten by The MEGA Life and Health Insurance Company, will continue to provide the services for Purdue students.
*Plan Choices:
Student Resources offers two plans designed specifically to meet the diverse needs of the International student population. The plan provides extensive coverage and includes a $350 per policy year deductible per insured for Preferred Provider and $700 for Out-of-Network Provider. To find out if there are hospitals or health care providers in your area who are part of the network, visit PHCS.
*Prescription Drug Benefit Processing:
A $15 co-payment applies to each covered prescription filled. When the Purdue pharmacy is used, this plan will pay 100% above the co-payment. Prescriptions must be filled at a CAREMARK participating pharmacy. Co-pay: greater of $15 or 30% coinsurance up to a $1,000 out-of-pocket maximum then co-pay greater of $15 or 10% coinsurance.

*Enrollment/ Payment Information:
To enroll for the year, please review the enclosed materials that include the Domestic Student Enrollment form. Premium may be sent directly to the Student Resources office in Dallas, Texas (as outlined in the brochure) or may be delivered to the Student Insurance representative located in PUSH Rm. 340 on the West Lafayette campus. Do not send the premium to the Bursar's office. Each enrollment form outlines two payment options:
- Check or Money Order: Premium paid by check or money order should be made payable to "Student Insurance".
- Visa/Master Card: Complete the VISA/MasterCard authorization form and send directly to Student Insurance.
*Waiver Information:
Students who elect to participate in a health insurance plan other than the plans offered by Student Resources must have an approved "Purdue University Health Insurance Waiver Form" on file prior to enrollment in University classes. The form is divided into two sections. Section one is to be completed by the student, while section two is to be completed by your insurance company's representative. The form can be found on the World-Wide Web by clicking here or by contacting the Student Insurance representative in PUSH Rm. 340 at 765-496-3998.
*For International Graduate Students who are employed half time or more as a Teaching or Research Assistant: The University offers a Graduate Staff Health Insurance Program for those students who are classified as Graduate Teaching or Research Assistants who have appointments of .50 FTE or greater for a semester or longer. Dependent spouses and children are also eligible for the plan; however, the University does not subsidize the cost of health insurance for spouses and dependent children.
