Palliative Care Team - Provides Consultation for hospitalized patients, to assist with support and symptom management for chronic and/or life-limiting illnesses. Also, assists with Goals of Care and Advance Care Planning. Lymphedema Therapy: Lymphedema is a swelling of a body part, most often an arm or a leg, caused by the abnormal accumulation of lymph fluid. Lymphedema therapy helps relieve this often painful swelling that can result from trauma, surgery, infection, disease or other factors. We have an experienced lymphedema team consisting of three nationally certified lymphedema therapists trained to treat any involved body part. Meals on Wheels of Greater Lafayette carries on a proud tradition of bringing hot and nutritious meals to individuals who have challenges getting meals on their own. Patient Navigation Services available for those diagnosed with Breast or Cervical Cancer once enrolled into the program. Cancer Wellness Program available as described in Nutrition and Wellness section. Funding provided for Mammograms, Pap tests and other diagnostic services/treatment related to breast and cervical cancers provided at no charge for eligible women. Referrals to volunteer wound care nurses that work with clients enrolled in our program to provide dressing changes and wound care assistance. Wound care supplies in limited quantities available to clients with breast cancer once enrolled in our program.
