Workplace Injury and Illness Reporting

Effective January 1, 2002, the Occupational Safety and Health Administration's (OSHA) injury and illness reporting requirements will change.  This will affect our reporting in three major ways: the form will change, the criteria for calling the injury in to Liberty Mutual will change, and the completed handwritten form must be sent to REM within a 48 hour time frame.

Memo from Director of Radiological & Environmental Management - This memo gives specific details about the changes that will take place. 

First Report of Injury Form - This form is available as an editable Microsoft Word Document that can be Word document may be e-mailed as an attachment to Kristi Evans in lieu of a hard copy.  This form is for all Purdue employees.

Training - This Power Point presentation explains the new procedure and helps supervisors recognize their responsibilities.
