Radioactive waste pickup requests need to be received by 5:00 pm on Monday for delivery Tuesday.  To ensure that waste will be picked up, make sure waste is free from contamination and all labels are completely filled out.   Use one of the following options to facilitate your pickup.

• WWW Submission Form - Our easiest option, just fill out the online form, submit, and you're done!  The deadline still applies for online submissions.
• Microsoft Word Form - Fill out this Microsoft Word form fax in to us at 49-47403 or send this via campus mail or email  to indicated addresses.  If you use Campus Mail make sure that you allow enough time so it arrives by Monday afternoon for Tuesday pickup.
