Direct Student E-mail Form (DSE)

Describe the student population to which the message should be sent.  Please be as specific as possible to avoid sending to students who have no need for the message.  Examples of population limits are: session, classification, gender and school.

Date Needed By (required)  Allow 5-7 business days to complete the request.  Depending on the complexity of the data requested, we may require more time.  Generally, messages that are not an emergency will not be accepted for immediate sending. The Office of the Registrar reserves the right to delay the sending of messages depending on the volume of work that is pending, especially during peak times such as the end of session.

Sent by: (Name) This name and e-mail address will display as the sender of the message.
(Email) These do not have to be the same as the requestor, but must be a University staff or faculty member.
Reply To: This e-mail address will receive any replies generated by the message.
Subject: This message heading will tell its viewer he purpose or content of the message.
