Enroll in Student Insurance
If you are a graduate staff (20 hours or more) or fellowship student, DO NOT purchase insurance in this manner.
|Step 1||On United Healthcare website, first select "Enroll Online Now" to start the enrollment process.|
|Step 2||Complete requested information.|
|Step 3||CAREFULLY select correct option. International students must select the International Plan policy 2013-261-4.|
|Step 4||Create an account. Remember your user name and password for future use. You will do this same process every year you are at Purdue. You will also use the same user name and password to access all communications from the insurance company.|
Enter name exactly as it appears in Purdue registration information.
Provide local United States address and Purdue email address when completing remainder of information.
ACCURATELY enter your PUID number (Purdue University Identification Number). Enter first 10 digits, including the first two zeros. Do not use dashes. Example: PUID given to you on the card or materials is 00123-45678 0. You should enter 0012345678.
|Step 6||Select payment method, accurately entering all information. (Mastercard, Visa, or Electronic Bank Draft) Please note that there is no additional fee when paying with a Visa or MasterCard (this applies to United Healthcare Student Insurance premium payments only). Two signatures are required for payment.|
|Step 7||PRINT a copy of confirmation, showing proof of payment, for your records. If no printer is available, save as pdf file and print later.|
|Step 8||Log off and then log back into account using password that you have created. Your Insurance card with assigned ID number will be ready to print after 48 hours. (Available after August 6th.) Please carry the card with you at all times.|
|Step 9||Print or save a copy of the policy brochure. Refer to brochure for policy and coverage information.|
To enroll for the INTERNATIONAL plan, go to http://www.uhcsr.com/purdue.
Enrollment must be completed by
September 8 (fall semester)
January 31 (spring semester).
Those international students who have not purchased insurance by the deadline will have a hold placed on their account and will incur a $200 late fee.
