Opt-out of the Graduate Staff Student Insurance

If you do not wish to participate in the Purdue University sponsored insurance program, you must take action to notify the university of this decision to opt out.

YOU MUST DO BOTH ACTIONS BELOW:

  1. All lnternational students must submit a waiver form to the Student Insurance Office to show proof of adequate insurance coverage. (Learn more)
  2. To opt out of the Graduate Staff plan, please click here to notify staff benefits. Please be prepared to provide your current health insurance information.
