Adding/Dropping Dependent Coverage
You are eligible to change your coverage only when you experience a qualifying event. The coverage change must directly relate to the qualifying event and must be requested within 31 days after the event occurs. Qualifying events can be found on the Grad Staff Insurance change form.
You must complete the Grad Staff insurance change form.
- Once the form is complete, return form to Staff Benefits, Frehafer Hall, 401 Grant Street, West Lafayette, IN.
- The appropriate payroll deduction changes will be reflected on your paycheck. (Summer enrollment changes must be paid as directed by Staff Benefits.)
