Enroll in Student Insurance
If you are a graduate staff (20 hours or more) or fellowship student, DO NOT purchase insurance in this manner.
|Step 1||On United Healthcare website, first select "Enroll Online Now" to start the enrollment process.|
|Step 2||Complete requested information.|
|Step 3||Domestic (US Citizen/Permanent Resident) students must select the Domestic Plan policy 2012-261-1.|
|Step 4||Create an account. Remember your user name and password for future use.|
Enter name exactly as it appears in Purdue registration information.
ACCURATELY enter your PUID number (Purdue University Identification Number). Enter first 10 digits, including the first two zeros. Do not use dashes. Example: PUID given to you on the card or materials is 00123-45678 0. You should enter 0012345678.
|Step 6||Select payment method, accurately entering all information. (Mastercard, Visa, or Electronic Bank Draft) Please note that there is no additional fee when paying with a Visa or MasterCard (this applies to United Healthcare Student Insurance premium payments only). Two signatures are required for payment.|
|Step 7||PRINT a copy of the confirmation for your records.|
|Step 8||Log off and then log back into account using password that you have created. Your Insurance card with assigned ID number will be ready to print after 48 hours. (Available after August 6th.) Please carry the card with you at all times. You will receive an email instructing you how to access your electronic insurance card. It can be printed or downloaded to your mobile device.|
|Step 9||Print or save a copy of the policy brochure. Refer to brochure for policy and coverage information.|
To enroll for the DOMESTIC plan, go to http://www.uhcsr.com/purdue.
Open enrollment period for the fall semester ends September 8.
Open enrollment period for the spring semester ends January 31
If the above deadline date falls on a weekend, the deadline is extended to the following Monday at 6 p.m. ET (Eastern Time).
After an open enrollment period ends, it is possible to still enroll. However, the effective date will become the date that the payment is made. There is no pro-rating of premiums. Visit the Student Insurance office in PUSH, Rooms 338/340 to enroll.
