Other Important Information

  • It is your responsibility to verify insurance coverage. If you do not see any payroll deduction for insurance, please contact the Staff Benefits Department as soon as possible (765-494-2222).
  • Replacement insurance cards can be requested from Student Resources Customer Service at 800-767-0700 or through http://www.uhcsr.com. Those students who are covered by Student Resource Health Insurance can access their insurance information online at any time. The site provides online access to coverage information, print-friendly replacement ID cards and claims status. If you have not yet created an account, just follow the “Create an Account” link in the College Student Login area of the homepage.
  • Terminating AY or FY appointment early may necessitate you to purchase continuation insurance in order to remain insured. Contact the insurance office at PUSH as soon as possible following a funding change.
  • Continuation coverage (post graduation) is only available if you have been covered under the policy for a minimum of 3 months. Please contact the Student Insurance office for more complete details. There is a brief window of opportunity to apply for continuation coverage, so prompt attention following termination is necessary.
  • If you experience a change in funding from a Graduate Assistantship to a Fellowship, it is vital that you contact the Student Insurance Office in PUSH, Room 338/340 so that you do not experience any lapse in coverage.
  • Vision coverage is included in your benefits package. Click here to learn more.
  • Dental coverage is a voluntary benefit administered by Delta Dental. Visit Purdue’s Voluntary Benefits Program to learn more. The site allows you to learn about the coverage and contact information. Graduate staff who are eligible for the Grad Staff Insurance Plan for the first time (after the open enrollment period) may elect the dental option within 30 days of their hire date.
  • International travel immunizations and counseling are not covered benefits.
  • Mental health services are not available for spouses at the PUSH facility, but are available at other in-network providers. Please review the brochure for all exclusions.
