Other Important Information
Timely payment is necessary to maintain insurance coverage. Failure to make renewal payments will cause insurance coverage to terminate and re-enrollment may not be possible. It is extremely important to make premium payments on time.
Continuation coverage (post graduation) is only available if you have been covered under the policy for a minimum of 6 months. Please contact the Student Insurance office for more complete details. There is a brief window of opportunity to apply for continuation coverage, so prompt attention following termination is necessary.
Vision coverage is a separate policy. The completed enrollment form and separate premium payment (check only) should be submitted directly to staff benefits in FREH hall. Further information on participating providers is available at the VSP website. Please note: The vision plan is not underwritten or administered by the United Healthcare Insurance Company.
Dental coverage is not available for fellowship students.
International travel immunizations and counseling are not covered benefits.
Mental health services are not available for spouses at the PUSH facility, but are available at other in-network providers. Please review the brochure for all exclusions.

Making Changes in Policy Coverage
You are eligible to change your coverage only when you experience a qualifying event. The coverage change must directly relate to the qualifying event and must be requested within 30 days after the event occurs. Please contact the Student Insurance Office if you have questions.
