Annual Purdue Supplement Amount = $1,394. This is paid through stipend check in two proportional amounts:
$564 for August - December and:
$830 for January - August
Total cost of insurance = $1,826 per year (1,346 Purdue) ($432 personal)

($546 Purdue stipend + $175 personal funds)

Eligibility
If you have received a Purdue Graduate Fellowship, you are eligible to enroll in a health insurance plan for you and your dependents.
Purdue University will include the portion of your stipend that pays for your insurance coverage in both August and December payroll checks. Enrollment and premium payment for a “student only” coverage will be on a semester basis. If you choose to enroll a spouse and/or children, you are responsible for paying the entire premium for these dependents. However, you have the option of paying on a monthly basis due to the higher costs of the premiums.
