Please note: Medical information is confidential and will not be discussed with a parent/guardian unless a medical release form has been signed by the patient. After the form is completed and signed by patient, please mail or fax (765-496-1227) to the Student Health Center, ATTN: Medical Records Releasing. If a claim has been filed to your insurance plan, we can discuss dates of service and amounts.
