Filing a Claim
A patient will be asked to sign an authorization form each time they visit PUSH. At that time, a box will be checked to either file charges to insurance, or to bill the patient directly.
Although tuition fees cover office visit charges for full time students at the Student Health Center, there are charges for:
- Laboratory services
- Physical Therapy
- Medications and supplies
- Special procedures
- Allergy shots
- Urgent Care
- Preventative care such as physical exams and women’s annual exams
PUSH will file to all commercial insurance carriers from which it can expect prompt payment.
PUSH is considered out of network for all insurance companies, except the university sponsored Student Resource insurance.
As a courtesy to our patients, PUSH will bill directly to commercial insurance companies for covered services when we have current insurance information on file.
It is important that you keep the health center informed of any insurance information changes. A student should carry a copy of the health insurance card at all times. Additional copies may be requested from your insurance company.
We do not bill to Medicare or Medicaid. If your insurance policy is with Anthem/Blue Cross/Blue Shield, we will file a claim at the patient’s request. However, a bill will be sent to the patient as this insurance company most often does not reimburse the University directly. Any amounts that are not covered by insurance are the responsibility of the patient.
It is very important for students to have a clear understanding of what the insurance company will and will not cover while the student is at Purdue University. It is wise to check with your insurance company to verify benefits before you receive services.
