Data Security Notice

The Purdue University Student Health Center (PUSH) has contracted with Fort Wayne Radiology since 2010, to interpret ("read") routine radiographic studies (x-rays) of its patients. On July 21, 2015, Fort Wayne Radiology notified PUSH that one of its subcontractors, Medical Informatics Engineering (MIE) experienced a data security compromise that affected protected health information relating to some of Fort Wayne Radiology's patients.

After investigation, Fort Wayne Radiology has learned that some of the patients to whom they have provided services are patients of PUSH. Individuals who received radiology services at PUSH during the period of time from August 1, 2010 to May 26, 2015, may be affected. The only information provided by PUSH to Fort Wayne Radiology and that may be affected by this compromise includes: Purdue University, patient name, a unique PUSH generated ID number, date of birth, the name of the exam being done with the images, the history as to why the exam is being done, the number of images, the ordering physician and any previously related studies. PUSH does not provide Social Security numbers, PUIDs, addresses, or insurance information to Ft. Wayne Radiology.

MIE's investigation is ongoing and any additional information concerning the types of affected information will be identified by MIE as it becomes known. MIE has provided written notice to affected individuals for whom MIE has a valid postal address. In order to reach those individuals for whom MIE does not have a postal address, Fort Wayne Radiology has provided notice on its website at: http://fwradiology.com/FWR-Website.pdf. Additionally, MIE has notified the Indiana Attorney General and certain other state and federal regulators of the incident, including the Office for Civil Rights.

MIE is offering affected individuals credit monitoring and identity protection services, free of charge, for the next 24 months. MIE has also established a toll-free call center to answer questions relating to this incident and the support and services being provided.
