2016-2017 Enrollment Information - Graduate Student Staff

Student Staff: Must be funded 20 hours/week or more; Fellowship Students being administered as graduate assistantship.

All graduate teaching or research assistants and graduate administrative staff employed .5 FTE 
or more are eligible to enroll in this insurance plan. All Fellowship students 
being administred as .5 FTE graduate assistantships are eligible to enroll in this plan.

Graduate Student Staff must enroll during the open enrollment period at the beginning of 
the Academic year or the university's premium contribution will be forfeited. If that occurs, 
the alternate plan option to obtain insurance would be to pay the full premium for either the 
domestic or international student plan.

New Graduate Student Staff hired after the enrollment period must enroll within 30 days of 
their start date. Insurance coverage becomes effective on the 6th of the month prior to your
start date.

Domestic Graduate Student Staff

Enroll through the Employee Portal for EBenefits: www.purdue.edu/employeeportal/
(From the Employee Portal, click the EBenefits link in the right-hand column of the Human Resources section. Sign on with your Purdue career account username and password.)

Print a copy of your enrollment confirmation.

Those Domestic (US Citizen/Permanent Resident) students who have taken no action to enroll by the deadline will forfeit the Graduate Student Staff insurance plan for the full academic year. The only other option available after the open enrollment period ends is to purchase the Domestic plan and pay the full cost of the premium for that plan. Coverage begins on date of payment.

Changes to your enrollment can only be made within 30 days of a qualifying event. Qualifying events are: marriage, divorce, birth, death, involuntary loss of insurance coverage. International Graduate Student Staff

Insurance enrollment is mandatory for International students.

Students who do not enroll by the deadline will lose their eligibility for the Graduate Student Staff plan. Since insurance is mandatory for international students, you will have to enroll in the regular International plan and pay the full cost. You will also be assessed a $200 late fee.

If you choose to waive the graduate student staff insurance plan, you must opt out and submit a waiver to the Student Insurance office by the deadline.
