2018 Anthem insurance cards to arrive soon with new ID numbers included; PayFlex replacement cards on the way

Human Resources would like to share when employees may expect to receive new insurance cards at their homes and important account and card changes for the new year.

Anthem

Employees soon will receive Anthem ID cards at their homes for each family member covered on a Purdue medical plan as well as for those who chose to opt out of Purdue medical insurance but elected one of the dental plan coverage options.

The one card will hold medical, prescription and dental information. It is important to watch for these cards -- which will include new ID numbers -- and replace the old cards with the new version.

Beginning Jan. 1, employees will need to provide their new ID card to their medical and dental providers and pharmacies. Information on current cards will not be valid after Dec. 31. The cards will have a new look and should make it easier to identify that the correct card and ID number is being shared with providers.

Because of the ID number change, employees previously registered with www.anthem.com will need to reregister accounts to be able to access medical and dental claim information beginning Jan. 1. Claims information dated Dec. 31 and prior can be accessed with their current login and will not be accessible from the new login.

Employees who do not receive their cards by Jan. 1 can contact Anthem directly at 855-502-6365.

PayFlex

Employees who are new to one of the following PayFlex accounts: health savings account, health care or limited purpose flexible spending account for 2018 and do not currently own a PayFlex card from a 2017 or prior account should begin receiving cards to their homes beginning Dec. 22. Only a card for the account holder will be sent.

Current cardholders with cards approaching their expiration date at the end of the month can expect to see cards arriving as early as this week. Cards are typically shipped 30 days before the expiration date listed on the card.

Employees should watch their mail at home for a discreet plain, white envelope with their new debit card inside.

Employees may request replacement and/or dependent debit cards by contacting PayFlex at 888-879-1454 or by logging into their account at www.payflex.com and selecting “Account Settings.”
