Center for Healthy Living shares information on accessing services

As the fall season arrives, the Center for Healthy Living (operated by One to One Health effective July 1) stands ready to help faculty, staff and covered dependents with their health and wellness needs. 

“The center continues to offer the same health and wellness services faculty and staff have come to expect -- now with an even more patient-focused approach,” says Candace Shaffer, interim associate director for benefits. “As with any transition, we have had to make a few adjustments along the way and want to take this opportunity to communicate some useful tips for accessing services at the center.”

“Staff at the center are eager for the start of the new semester and helping the Purdue population get acquainted, or in some cases reacquainted, with the center and the services and support they can provide,” says Jennifer Hodges, Center for Healthy Living’s director of operations. “Employees and dependents of employees covered by a Purdue medical plan have the option to schedule appointments by phone or through our patient portal. There are many benefits to setting up the portal account and employees are encouraged to call the center for assistance with setting up their account for the first time.”

The Center for Healthy Living’s website portal offers many convenient features:

* Schedule and review appointments.
* Receive appointment reminders and other Center for Healthy Living emails.
* Access incoming provider messages.
* View visit summary.
* Choose types of communications you receive.
* Dependent spouse and children of all ages have the option to open their own account.

Portal account setup

Employees and dependents can follow these steps to create their patient portal account:

* Call the CHL at 765-494-0111 to activate account.
* Wait to receive an email from the Center for Healthy Living (check spam filter if email is not received in a timely manner).
* Follow the link in the Center for Healthy Living email.
* Enter the username and password provided in the email.
* Once logged in, you will be prompted to reset your password.

Appointments

Appointments may be made by calling the center at 765-494-0111 or scheduling online through the patient portal. To schedule online, first-time portal users need to contact the center to establish their portal account.

Employees who registered before July 1 will need to call the center to establish their account as a first-time user since this portal is new with the transition to One to One Health.

At this time, employees scheduling appointments online may choose morning or afternoon and should expect a call from the center to set an appointment time within that time frame. Employees also have the option to choose a provider they prefer to see. Currently, two physicians are listed with more providers and health coaches being added over the next few weeks.  

Forms available online

In addition to the portal access, employees have the option to complete the patient questionnaire before their appointment and bring it to their first visit. Employees who wish to receive personal copies of their medical records from the Center for Healthy Living from visits before July 1 may complete the authorization to use or disclose protected health information form.

About the center

The center is open 7 a.m. to 6:30 p.m. Monday through Thursday and 7:30 a.m. to 5 p.m. Friday. Lab hours are 7 a.m. to 4 p.m. Monday through Friday. Visit the center online to learn more about the services available.
