Deadline to complete physicals and biometric screenings as part of incentive program is Thursday

There's still time to take part in the Center for Healthy Living's incentive program. To promote healthy living, the Center for Healthy Living on Purdue's West Lafayette campus is offering a physical and biometric screening for benefits-eligible faculty and staff and covered adult dependents. Chances to earn free prizes courtesy of Premise Health, current Center for Health Living vendor management company, are still available to those who: 

* Submit confirmation of a physical with biometric information by Thursday (June 15) -- the weekly winner receives a $20 Amazon gift card (eight total winners).

* Sign up for no-cost wellness services -- including health coaching, employee assistance counseling, medication therapy management and more -- at the Center for Healthy Living by Thursday (June 15) – each biweekly winner receives a three-month membership to the Cordova Recreational Sports Center (four total winners).

Physicals with biometric screenings provide important data on a variety of health levels that helps employees to better focus on their overall health and wellness. During a biometric screening, blood glucose level, body mass index, waist circumference, blood pressure and cholesterol are assessed, providing feedback useful in identifying possible health risks.

“We hope the incentive program helps encourage our employee population to have their physicals and get their biometric screenings done at that time,” said Candace Shaffer, interim associate director in benefits. “Employees who know their numbers are better positioned to take care of their health and also utilize the no-cost wellness programs offered at the Center for Healthy Living that fit their specific needs.”

An annual physical is the prime time to have biometric data collected. Employees are encouraged to get their annual physicals and biometric screenings wherever it makes the most sense for them to do so.

In order for the Center for Healthy Living to utilize an employee’s biometric data and count the employee eligible for the incentive program, employees should do the following:

1. Visit their provider for an annual physical -- covered at 100 percent for those covered by a Purdue health plan.

2. Ask their provider to complete both the biometric screening and authorization to use or disclose protected health information forms from the Center for Healthy Living.

3. Return both forms (available through links above or on the center’s website) to the Center for Healthy Living or ask the provider to submit both forms directly via secure fax at 765-496-6656.

The center’s services are offered to all benefits-eligible faculty and staff, including those who opt out of Purdue medical coverage, and their covered adult dependents. Preventive care -- including annual physicals -- and wellness workshops are among the services offered at no charge.

Preventive care is covered at 100 percent by all Purdue medical plans, when an in-network physician is providing the care. The center’s staff works collaboratively with outside providers to help ensure patients get the best overall care including supplemental, complementary care available at the center. 

All data will be collected by Premise Health. Purdue will not have access to individual data.
