Delta Dental cards to arrive soon; Anthem cards sent only to those who made plan changes for 2016

Human Resources would like to inform employees that their Delta Dental ID cards will arrive soon. There was a delay in the distribution of dental cards; however, they should begin to arrive to employees' homes over the next few days.

Cards will be distributed to all employees enrolled in a Purdue medical plan as well as those who chose to opt out of Purdue medical insurance but elected one of the additional dental plan coverage options.

As a reminder, effective Jan. 1, all benefits-eligible employees and dependents covered under a Purdue medical plan automatically receive preventive dental coverage. Coverage includes two cleanings, a fluoride treatment (up to age 19) and standard X-rays up to a $500 per person maximum through Delta Dental at no additional cost.

Employees will receive two perforated cards in an envelope labeled with the Delta Dental logo. The cards will be preprinted with the employee's name, Purdue University as the client name, the group and subgroup number and the employee's Purdue ID number. If additional cards are needed, they may be printed by logging into Delta Dental's Consumer Toolkit on its website and selecting "Print ID Card" from the left-hand side of the screen or by calling 800-524-0149.

Cards will be generated only for employees. Delta Dental has combined its family unit and individual plans under the employee's record. Delta's system is set up to process claims on the type of coverage each family member has; however, all claims for family members should be filed under the subscriber's nine-digit Purdue ID number (minus the first zero and do not include the trailing version number).

For dental appointments scheduled before receiving a card from Delta Dental, the nine-digit Purdue ID number may be provided before the appointment. Employees may complete a claim form for reimbursement of any dental services performed and paid for before receiving the Delta Dental card.

Anthem ID cards -- for medical and prescription claims -- were sent only to employees who made plan changes for 2016. All other employees will continue to use their existing ID cards. Anthem sends ID cards for each family member covered on a Purdue medical plan.
