All employees must certify tobacco-user status to avoid paying higher 2015 medical premium

An additional medical insurance premium of $500 or $1,000 awaits employees who fail to certify their tobacco-user status during this year's benefits open enrollment.

Employees are required to certify their status annually during open enrollment, even if they're keeping the same medical plan they've had in the current year.

Status options are:
1. Non-tobacco user.
2. Tobacco user.
3. Tobacco user who has completed an approved cessation program between Sept. 1 and Nov. 14, 2014.

Those who do not log in to EBenefits and certify will be considered a tobacco user and will pay the $500 per person tobacco-user additional premium that applies to all Purdue medical plans. If both the employee and the spouse/same-sex domestic partner are covered, the additional premium will be $1,000.

To certify tobacco-user status or enroll for 2015 benefits, employees go to the Purdue Employee Portal, follow the EBenefits link at the top of the page, and enter their Purdue career account username and password to begin the process.

Employees have until 6 p.m. (ET) Friday (Nov. 14) to certify their tobacco-user status and enroll for 2015 benefits.
