Center for Healthy Living offering $30 sports physicals

Sports physicals are available at the Center for Healthy Living for just $30 for dependents covered on a Purdue medical plan. For added convenience, faculty and staff may use their PayFlex debit cards to pay for the sports physical from their health savings account (HSA) or health care flexible spending account (FSA).

Sports physicals are not eligible on the Purdue medical plan, so they won't count toward the employee's deductible or out-of-pocket maximum.

"Everyone getting a sports physical will need to bring the required IHSAA form to the appointment," says Millie Fenneman, practice administrator at the center. The form is available at www.ihsaa.org/Portals/0/ihsaa/documents/quick%20resources/physicalform.pdf.

Call 49-40111 to make an appointment. If the sports physical will be the child's first visit to the center, Fenneman has a time-saving tip: Complete and print the patient intake form at www.purdue.edu/hr/CHL/pdf/patientIntakeandWellWomanQuestionnaire010512.pdf, and bring it to the appointment.

"Remember, a sports physical does not replace an annual preventive exam for your child with their regular pediatrician," Fenneman says. "In your child's best interest, you need to get both."
