Completing form in advance speeds up first visit at Center for Healthy Living

Clients who are about to go to their first appointment at the Center for Healthy Living may now complete the customary patient questionnaire beforehand. The form asks for general information, such as the client's name, address and age, and then gathers the client's health history.

Clients can save time at the center by completing the form before arriving for their appointments. The form is required only for the first visit.

* Clients may print the form from the center's website, complete it by hand and then bring it to the first appointment.

* Clients may complete and submit the form electronically through the Center for Healthy Living patient portal at https://purdue.opticonsite.com. The portal calls the questionnaire the "intake form."

For links to the form and instructions, clients should go to www.purdue.edu/healthyliving and then click the "Complete patient questionnaire" link in the Things to Do box on the left side of the page.

Anyone with questions may call the Center for Healthy Living at 49-40111. The center is in the Purdue West Shopping Plaza at 1400 W. State St.
