Tier 1 lab on campus offers quick, easy service

The Tier 1 lab at the State Street Office Facility (SSOF) on the West Lafayette campus is set up to make trips to the lab fast and convenient.

* The lab serves only employees and dependents covered by a Purdue medical plan. Because it's not open to the public, patients often get service without waiting.

* Patients can park right outside the door in the onsite parking lot. Designated customer spaces provide easy parking, even for family members who don't have a Purdue parking permit.

* Patients may walk in for service or may call 49-41872 for an appointment.

* Lab hours are designed for convenience, offering service before 8 a.m. and during the traditional noon lunch hour. Hours are 7 a.m.-5 p.m. Monday-Thursday (closed Fridays and University holidays). The lab is closed for lunch at 1-1:30 p.m. Monday-Thursday.

The SSOF facility is a Mid America Clinical Laboratories site at 1601 W. State St., west of Airport Road. WorkLife Programs is also in SSOF.

Because it is a Tier 1 lab, employees and dependents covered by the Purdue Incentive or Purdue Copay medical plans pay nothing for services at the SSOF facility.

Choosing a Tier 1 lab does not affect benefits under the Choice Fund with HSA medical plan; however, participants may still take advantage of the very cost effective pricing provided by these labs.

In addition to saving money for themselves, covered employees and dependents using the SSOF facility, or any Tier 1 lab, save money for the Purdue medical plans.

Patients should bring their Purdue medical plan ID card when going for a lab test.

Health care providers may fax a lab order to the SSOF lab at 765-474-8460 or patients may bring a signed, written order when they come to the lab. Physicians should include their fax number on the lab order if they wish to have the results faxed to them.
