SSOF Tier 1 lab on campus offers quick, easy service

The Tier 1 lab at the State Street Office Facility (SSOF) on the West Lafayette campus is open for business and set up to make trips to the lab fast and convenient.

* The lab serves only employees and dependents covered by a Purdue medical plan. Because it's not open to the public, patients often get service without waiting.

* Patients can park right outside the door at the onsite parking lot. Visitor spaces provide easy parking even for family members who do not have a Purdue parking permit.

* Patients may walk in for service or may call 49-41872 for an appointment.

* Lab hours are designed for convenience, offering service before and after the common 8 a.m.-5 p.m. work schedule and during the traditional noon lunch hour. Hours are 7 a.m.-5:30 p.m. Monday-Thursday (closed Fridays and University holidays). The lab is closed for lunch at 1-1:30 p.m. Monday-Thursday.

Employees and dependents covered by the Purdue Incentive or Purdue Copay medical plans pay nothing for preventive or diagnostic lab services at the SSOF lab.

Choice Fund with HSA medical plan participants pay nothing for preventive lab services at SSOF. Choice Fund patients pay for diagnostic lab work at SSOF according to their plan's deductible and coinsurance arrangement, but will still benefit from the lab's cost-effective pricing.

The SSOF facility is a Mid America Clinical Laboratories site at 1601 W. State St., across from Horticulture Park. WorkLife Programs is also in SSOF.

As with other labs, patients need a written and signed lab order from their health care provider. Health care providers may fax the lab order to the SSOF lab at 765-474-8460 or patients may bring the order with them when they come to the lab. Patients should also bring their Purdue medical plan ID card.
