Employee Assistance services available
Benefits-eligible employees have access to free, on-site counseling and referral services through WorkLife Programs Employee Assistance.
Employee Assistance counselors are available to help employees address a wide range of issues, whether they relate to an employee's personal life, family or job.
Employee assistance services also are available to official retirees, dependent family members, spouses/same-sex domestic partners, and families of deceased employees.
WorkLife Programs Employee Assistance is located at the State Street Office Facility (SSOF) at 1601 W. State St. To make an appointment for an in-person or telephonic consultation, call 49-47707.
For those who prefer telephonic services or have after-hours needs, employee assistance services are available 24 hours a day, seven days a week through CIGNA's Choose Well, Live Well Personal Health Team. To contact the Choose Well, Live Well Personal Health Team, call 800-767-7141 and select option 2. Online resources are available. Log on to www.cignabehavioral.com, enter your employer ID (purdue), search the provider directory, and request a referral.
