Tobacco-User Additional Premium

Approved Tobacco Cessation Programs

Tobacco users who complete an approved tobacco cessation program between Sept. 1 and Nov. 24, 2011, qualify for a waiver of the 2012 tobacco-user additional premium. Campus Options

Quit Smart (Classroom option for West Lafayette)

You must enroll in and complete this in-person program by attending at least five of six classes in the program.

  • Call WorkLife Programs at 765-494-5461 or visit the WorkLife Programs home page to register for classes in October.
  • Available to West Lafayette campus employees and their covered spouses/same-sex domestic partners.

Quit for Good! (Classroom option for Calumet)

You must enroll in and complete this in-person program by attending at least five of six classes in the program.

  • Visit the Purdue Calumet wellness page for dates, times and registration information.
  • Available to Calumet campus employees and their covered spouses/same-sex domestic partners.

Cigna Option

Quit Today (Online)

You must complete the program through Step 6.

  • Call 800-767-7141 to get started. Enroll no later than Oct. 1 to allow time to meet the Nov. 24 deadline.
  • Available to employees and spouses/same-sex domestic partners at all campuses who are covered by a Cigna medical plan option through Purdue.

Other Options

Community Alternative

If you take part in a tobacco cessation program through other campus resources, hospitals or community organizations, you may qualify for waiver of the tobacco-user additional premium, depending on the specifics of the offering.

Before taking part, contact Human Resources WorkLife Programs at 765-494-5461 to review the details of the offering and find out if completion of the program will qualify you for waiver of the additional premium. If the program qualifies, WorkLife Programs will provide you with a Community Alternative form that you must have signed by the community program's representative when you have finished the program.

Medical Alternative

If you are unable to participate in a traditional program due to a medical condition, contact WorkLife Programs at 765-494-5461 to develop an alternative way to qualify for waiver of the tobacco-user additional premium.
