Support employee participation in Healthy Purdue

 


Healthy Purdue, the Purdue health initiative, is back for 2007, and its success depends on employee participation. Encouragement from supervisors is key to achieving this goal.  

How to take part

Participants can choose to complete a wellness screening on campus or with their health care provider. All of Purdue’s medical plan offerings include free network lab services. In addition, the Incentive PPO, Purdue 500, and Arnett Managed Care plans include free in-network preventive care, up to a specified amount.

Campus wellness screening dates are available through the School of Nursing and with StayWell Health Management. Participants can register online for a screening.

For those participants who choose to complete a wellness screening with their health care provider, a screening results form is available.

Once the wellness screening has been completed, participants will fill out a HealthPath® Questionnaire (HPQ). The questionnaire will be available between December 1, 2006, and February 28, 2007, and participants may choose to complete it online at the Healthy Purdue Web site or in paper format.

Those choosing to complete a paper HPQ must request it before February 14, 2007, in order to have enough time to complete it by the February 28 deadline. They will receive their results in the mail within five business days of StayWell receiving their questionnaire booklet.

Further instructions for completing the HPQ are available in the Action Packs that were mailed to employee homes. The Action Pack is also available online. Once an employee has completed a wellness screening and the HPQ, they will receive a before-tax incentive of $150 in their paycheck. Spouses and same-sex domestic partners are also eligible to participate, and their incentives will be processed on the Purdue employee’s pay.

For a spouse or same-sex domestic partner to participate in Healthy Purdue, the Purdue employee must complete and mail a spouse verification form, available in the Action Pack or online.

Eligibility

To be eligible for Healthy Purdue 2007, employees must be faculty or staff classified as benefit-eligible and have been hired on or before December 1, 2006.

Participation

Participation in Healthy Purdue is supported by University administration. We ask you to support participation actively by allowing your employees time away from their jobs in "worked regular" pay status to take part in an on-campus wellness screening and to complete the HPQ. We encourage staff who work evening and night shifts to participate, too. Although shift employees will not be in pay status if they participate during times outside their normal work schedule, you may consider providing scheduling flexibility to accommodate these employees.

The expected time to complete a wellness screening on campus is about 30 minutes. Wellness screenings are scheduled at various buildings on campus so that staff can attend the one closest to where they work. The HPQ takes about 30 minutes to complete.

New employees

Benefit-eligible employees hired after December 1, 2006, may participate in a campus wellness screening in 2007. New employees are not eligible for the HealthPath® Questionnaire, $150 before-tax incentive, health coaching, and the $250 wellness achievement incentive until 2008.
