Worker's comp form updated, simplifiedThe First Report of Injury Form has been revised, and the Purdue ID is no longer needed. The new version of the form asks for the employee's department number and date of hire. Feedback from departments about the new electronic forms has been very helpful. Departments report that they appreciate the simplicity of the forms.
