Carolyn B. Cooper, M.D., ABFM, CCD
Dr. Carolyn B. Cooper is a board certified family physician with 17 years clinical experience. Dr. Cooper is also certified as a clinical densitometrist, a specialty dedicated to the diagnosis and management of osteoporosis. She received her MD at the Medical College of Virginia and completed family practice residency training at Methodist Hospital in Indianapolis.
Dr. Cooper has spent her career in private practice at American Health Network in Lafayette. In 2012, Dr. Cooper received recognition for the delivery of quality diabetes care from the National Committee for Quality Assurance. She is a member of the American Association of Family Physicians, American Medical Association and the Indiana State Medical Association.
Dr. Cooper is familiar with the Purdue culture, as her husband is a metabolomics applications specialist at the Bindley Bioscience Center at Purdue's Discovery Park. Additionally, Dr. Cooper is the music minister/organist of Chapel of the Good Shepherd, the Episcopal Campus Ministry at Purdue. She volunteers with the Commemorative Air Force, Food Finders Food Bank, Tippecanoe County 4-H and Lafayette School Corp. In her free time, Dr. Cooper and her husband are amateur home winemakers. She and her husband have two children, ages 16 and 13.
Edward Hoskins, MD
Edward J. Hoskins, MD, PhD, is a board certified physician in family practice with 31 years of clinical experience. For the past 18 years he served Purdue students at the Purdue University Student Health Center (PUSH). During that time he acted for 14 years as the PUSH quality management coordinator as a board certified physician in quality assurance. In 2006, Dr. Hoskins was voted the Physician of the Year by the American Academy of Specialists in Family Practice. Dr. Hoskins has had multicultural experience having lived and worked short- and long-term in several different countries in the Middle East and South Asia; he speaks Spanish and Arabic. Dr. Hoskins is available intermittently at the Center for Healthy Living for regular provider coverage.
Stuart Isaacson, MD
Dr. Stuart Isaacson is a board-certified family physician with more than 25 years of broad clinical experience. Serving as medical director of Concentra’s primary care / urgent care center in New Berlin, Wisconsin, from 2003 to 2011 and then working with Wheaton Franciscan Hospital in Racine, Wisconsin, Dr. Isaacson enjoy all aspects of primary care as well as occupational medicine.
Dr. Isaacson spent four years with US Public Health Service caring for underserved patients in rural physician shortage areas and also worked with the military at both Fort Hood in Killeen, Texas, and Fairchild Air Force Base in Spokane, Washington.
In addition to medicine, Dr. Isaacson has a wide range of interests including writing, film, travel and history. He has a broad background in medical publishing and editing and served on the board of the American Medical Writers Association. He also studied film theory and criticism. Dr. Isaacson has two children – a son who is also a physician and a daughter who works in interior design.
Debra Crume-Nagel, RN, BSN, CHWC
Debra Crume-Nagel has been in nursing for 25 years. Upon graduating from Ball State University with a BS in nursing, she worked for St. Vincent Hospital in Indianapolis for 15 years. She started at St. Vincent Carmel Hospital on the medical/surgical floor, and later transferred to the ICU department at St. Vincent Hospital on 86th St. in Indianapolis.
After moving closer to family in Carroll County, she was employed by St. Elizabeth Hospital in the CCU/PCU department. Finally landing in Rensselaer, Debra worked at Jasper County Hospital as a patient care coordinator, which involved moving throughout the hospital and assisting in all nursing departments.
Debra and her husband Duane have six children with ages ranging from 10 to 29. They enjoy volunteering at St. Augustine Church and spending time with their family.
Cheryl A. Laszynski, RN, BSN, MBA, CHWC
Cheryl has 28 years of health care executive/management experience. She received a BS in nursing from Indiana University and an MBA from Indiana Wesleyan University. As the administrator/director of ancillary services for Arnett HealthSystems, she managed a variety of physician practices and clinical departments, such as urgent care centers, occupational medicine, pharmacy, lab, dietetics, and physical/occupational/speech rehabilitation.
Cheryl's clinical experiences include emergency/intensive care, home health care, managed care/benefit administration and quality improvement. Cheryl will look familiar to Purdue employees, as she has worked on campus for the past eight years. Most recently, she was the worklife administrator for WorkLife Programs.
Cheryl is married and has two adult children. She enjoys running, cycling, reading and spending time with her family.
Suzanne Rossiter has been a critical care registered nurse (RN) for eight years. She has worked at Jasper County Hospital in Rensselaer, Ind., and Methodist Hospital in Merrillville, Ind. Suzanne graduated in 2004 from Purdue University with a BS in nursing. She earned her master's in nursing education and graduated magna cum laude from Valparaiso University in 2011. Suzanne also has experience as a diabetes educator.
She and her husband have 3-year-old twin girls.
Tia is a firm believer in continuing education and self-improvement. She started her career as a licensed practical nurse and later returned for her associates in nursing to attain her registered nurse (RN) license. She graduated from Purdue University Calumet in 2012 with her bachelor of science in nursing. Tia maintained Dean’s List status for all three degrees. Tia has over 13 combined years of nursing experience, including labor and delivery at Home Hospital and a variety of roles at Indiana University Health Arnett within departments such as primary care, specialty, pre-operative and recovery unit. Tia has also served as a care coordinator for IU Health Arnett’s patient centered medical home department.
While at IU Health Arnett, Tia attained her LEAN yellow belt from Purdue University and a Daisy Award for exceptional nursing care. Most recently, she has worked as an RN care manager with Evolent Health, a company who focused on IU Health’s employees and Medicare Advantage populations to assist them with navigating the healthcare system, advocacy and managing their health and wellness needs.
Tia has three adult children, including a set of twins, and two grandchildren. She enjoys spending time with her family and friends; her Catahoula, Eddie; reading; attending concerts; and running.
Jessica Dicks has more than 10 years of administrative assistant/front office experience and three years of medical assistant (MA) experience. In 2010, Jessica received her associate's degree in applied science from Harrison College in Fort Wayne. Jessica previously worked at Unity Healthcare Internal Medicine in West Lafayette.Jessica is married and has two daughters, ages 11 and 7.
Kathy Dill has 30+ years of medical assisting and phlebotomy experience. Kathy grew up in Bakersfield California and moved to Lafayette area approximately 15 years ago. She is married and has 2 grown children. Kathy enjoys playing and hanging out with her 2 ½ year old granddaughter. She also enjoys going for a ride on their Harley.
Sarah Metcalf graduated in 2012 has four years of medical assisting experience and four years of phlebotomy experience. Two the four years of her medical assisting career were spent in Cardiology.
Sarah was born and raised in Chicago, IL and moved to Lafayette when she was 11 years old. She has to daughters, ages 2 and 5. In her free time, she enjoys deer hunting, hiking and spending time with her father.
Nancy Cook, BSN, MSW, LCSW
Nancy brings more than 35 years of experience in the medical and mental health field to the Center for Healthy Living. She holds degrees from both Purdue and Indiana universities. In 2010, Nancy graduated with honors from the Masters of Social Work Program at Indiana School of Social Work. Most recently, Nancy was a medical/psychiatric social worker at Franciscan St. Elizabeth Hospital, where her job included psychiatric assessments, counseling individuals, couples and families, running educational groups, actively participating in patient care plan development and implementation and planning for discharge. Nancy completed mediation training and registration with the State of Indiana in 2014. As a nurse, Nancy has a background in adult and child psychiatric assessment and care in a hospital setting. She also worked as a school nurse working with many issues including adjustments and behavior issues. Other experiences include additional training in play therapy and working with children and adolescents. Nancy has affiliations with both the local and state National Association of Social Workers.
In her free time, she enjoys spending time with her family, riding her horse, walking her dogs, Purdue athletics and reading. She has lived in Lafayette with her husband, Steve, a local periodontist, for 30 years and loves the community. They take great pride in their two adult sons, their sons’ significant others and their two grandsons. Continuing to grow and learn is one of Nancy’s greatest joys.
Lindsey Millburg, MSW, LSW
Lindsey graduated with honors from the Indiana University School of Social Work in 2014. She was most recently employed as a school social worker with the Tippecanoe School Corporation. Lindsey may look familiar to some as she worked at Purdue for six years in residential life at Harrison Hall. She has experience with medical/psychiatric social work when previously employed at St. Elizabeth East Hospital. Prior to St. Elizabeth, Lindsey was an adult and adolescent outpatient therapist at Cummins Behavioral Health Systems, Inc. She is certified in Therapeutic Crisis Intervention and is a Question, Persuade, Refer (QPR) Instructor. QPR is a program that teaches skills to recognize signs and symptoms of suicide and how to persuade and refer that person to get help. She enjoys working with all age ranges, from children to geriatric, and has worked with couples, families, groups and individuals.
Lindsey is married to Dave, an Indiana State Trooper, and enjoys spending time with him, her stepdaughter, their three dogs (including one police K-9) and three cats. They enjoy going hiking, fishing, camping and taking road trips to see family.
Laci Parcell, MSW, LSW
Laci Parcell has over 10 years of experience working with families, mental wellness, children and the aging population. Laci graduated with honors from IUPUI in 2014 with a Master’s Degree in Social Work. She also has a bachelor's degree in Journalism from Indiana University. Laci moved to Lafayette in 2014 and worked at Indiana University Health Arnett as a social worker/discharge planner. She is experienced in psychiatric assessments, facilitating therapeutic groups, discharge planning, implementing treatment plans and behavioral plans and has training in motivational interviewing. Laci has previous experience as a therapist in a community mental health facility as a family case manager for the Department of Child Services, teaching activities of daily living and as a prevention educator. She enjoys working with children, families, individuals and groups. Laci is married and has one child. She enjoys reading, learning new things, learning more about the Lafayette area and visiting family in southern Indiana.
Ellen Welch, MS, RD, CD, CHWC
Ellen Welch graduated from Purdue University with a BS in dietetics in 2011. She went on to complete her internship through the Purdue Coordinated Program in Dietetics. Ellen worked at Franciscan St. Elizabeth Health hospital for three years, where she provided nutritional counseling and care plans for patients with various diseases, both acute and chronic. She is currently pursuing a master’s degree with a concentration in public health from Indiana State University.
Ellen enjoys running, cooking, reading, watching the Indianapolis Colts and all things Purdue. She and her husband have been married for two years and have a chocolate lab named Harry.
Kristin Henrich, PharmD
Kristin Henrich graduated from Butler University in 2011 with her doctorate of pharmacy. Since her graduation she has gained experience as a clinical pharmacist in the community hospital setting, previously working at Indiana University Health Arnett. She has experience with multiple disease state management, some of which include diabetes, hypertension, and anticoagulation. She has first-hand experience with treatment of acute illness and an appreciation for the vulnerability and stress that it puts on patients and their family members. In addition, Kristin has experience with health/wellness screenings, patient education and developing individualized patient care plans and working with interdisciplinary teams to achieve optimal patient outcomes.
Kristin will be providing Medication Therapy Management (MTM) and Collaborative Drug Therapy Management (CDTM) services at no cost for benefits-eligible Purdue employees and their dependents covered on a Purdue medical plan.
Tamaira K. Ridge, Health Center Manager
Tammy has 16 years experience in healthcare administration. She holds a bachelor of science in business management, earned in 2012 from Indiana Wesleyan University. In addition, she is experienced in Lean practices, having attained her yellow belt from Purdue University and a bronze certification in Lean transformation from Simpler. During her career with Indiana University Health, she managed a wide variety of physician practices including dermatology, plastic surgery, pain management, rheumatology, orthopedics, neurology, sports medicine and physical therapy. She also managed two major service lines, musculoskeletal and neuroscience, which expanded services to other communities. Tammy enjoys work in Lean transformation and has served on two major healthcare value stream committees which oversaw several departmental Lean initiatives for healthcare improvements, focusing on the patient experience and related outcomes.
Tammy has three adult children and two grandchildren. She enjoys running, working out, wildlife and supports animal causes.
Erika Burchett was born in Terra Haute, IN. She moved to Lafayette around the age of six until around 10 when she moved to AZ. She lived there until 2003 when she moved back to Lafayette IN. She graduated McCutcheon High School in 2006. She went to Harrison College and graduated with an Associate’s Degree in Medical Assisting in 2008. She has a long-standing background in customer service. She currently lives in the Pine Village area with her husband, two children and two dogs.
Tammy Gentry graduated from Indiana Business College (Harrison College) with an associate’s degree in office management. She has more than 15 years of experience in human resources, payroll and employee benefits. Most recently, she worked as a patient access specialist at a family medicine, dermatology and sports medicine office within the Franciscan Physicians Network.
Tammy has four children and resides in Lafayette. She enjoys camping, bowling and spending time with family and friends.
Maggie Rice has 13 years of experience an administrative assistant and receptionist. She has worked in health care at various medical offices including the Danville Care Center, Dr. Conn Dental, and, most recently, Carle Physicians. Maggie specialized in computer programming at Danville Area Community College in Danville, Ill.
Maggie recently moved to West Lafayette from Georgetown, Ill. She has two boys, ages 12 and 18. In her free time, Maggie enjoys hiking, canoeing and boating.
