Primary Care Providers

Carolyn B. Cooper

Carolyn B. Cooper, M.D., ABFM, CCD

Dr. Carolyn B. Cooper is a board certified family physician with 17 years clinical experience. Dr. Cooper is also certified as a clinical densitometrist, a specialty dedicated to the diagnosis and management of osteoporosis. She received her MD at the Medical College of Virginia and completed family practice residency training at Methodist Hospital in Indianapolis.

Dr. Cooper has spent her career in private practice at American Health Network in Lafayette. In 2012, Dr. Cooper received recognition for the delivery of quality diabetes care from the National Committee for Quality Assurance. She is a member of the American Association of Family Physicians, American Medical Association and the Indiana State Medical Association. 

Dr. Cooper is familiar with the Purdue culture, as her husband is a metabolomics applications specialist at the Bindley Bioscience Center at Purdue's Discovery Park. Additionally, Dr. Cooper is the music minister/organist of Chapel of the Good Shepherd, the Episcopal Campus Ministry at Purdue. She volunteers with the Commemorative Air Force, Food Finders Food Bank, Tippecanoe County 4-H and Lafayette School Corp. In her free time, Dr. Cooper and her husband are amateur home winemakers. She and her husband have two children, ages 16 and 13.

Dr. Michael Summers

Michael L. Summers, M.D., ABFM

Dr. Michael L. Summers is a board certified family physician who enjoys all areas of family medicine. He’s practiced family medicine in central Indiana since 1988. A Butler University graduate, he received his MD from Indiana University School of Medicine and completed his residency in family practice at Community Hospitals of Indianapolis.

Originally from Greenfield, Ind., Dr. Summers has practiced medicine with Community Hospitals and Hancock Regional Hospital. He also served as a medical director for several years. The Connelly Group and Indianapolis Monthly Magazine recognized Dr. Summers as a top physician in 2011, 2012 and 2013. He is board certified in family medicine and a member of the American Academy of Family Physicians and the Indiana State Medical Association.

Dr. Summers has always been a big fan of Purdue University and has family living in the area. He volunteers for several organizations, and he served eight years as a school board member for Greenfield Central Schools. In his free time, Dr. Summers enjoys traveling, sports, spending time with his wife, three daughters and his grandson, and taking care of their horse and two giant schnauzers.

Teri Gee, MSN, FNP-BC

Teri Gee, MSN, FNP-BC

Teri Gee has more than 28 years of nursing experience. She graduated in 2001 from Purdue University with her master’s in nursing and her family nurse practitioner.

Throughout her career, Teri has worked in family medicine, internal medicine, women’s health and emergency medicine. She’s also taught nursing.

Teri loves spending time with her family and friends. She has two daughters and one son. Teri enjoys traveling, exercising, shopping, cooking and gardening.

Kimberly K. Jefvert, MSN, FNP-BC

Kimberly K. Jefvert, MSN, FNP-BC

Kim Jefvert has more than 24 years of experience as a nurse practitioner. Beginning her career as an adult nurse practitioner (ANP), she became board certified as a family nurse practitioner (FNP) in 1995. She has completed the American Academy of Anti-Aging's functional medicine fellowship. 

Throughout her career, Kim has worked in schools of nursing, an employer health clinic, community health clinics, rural health primary care clinics, prison health care and pediatric private practice.

Kim volunteers at the Benton County Community Foundation and supports her 16-year-old daughter's riding career at Tall Pine Stables. She has a son who graduated from the John Deere Technology Program. In her free time, she enjoys gardening, swimming, reading and biking.

 

Beth Ann Taylor

Beth Ann Taylor, APRN, MSN, FNP-BC, GNP-BC

Beth Ann Taylor has more than 25 years nursing experience. She received her RN in 1988 and her BS in nursing in 1993. Beth is triple certified as an adult nurse practitioner, a family nurse practitioner and a geriatric nurse practitioner. Throughout her career, she has worked in hospital, community health center, private practice and home health settings.

In her free time, Beth enjoys playing with her three dogs, volunteering with the 4-H Club group Pawprint, riding her bike and gardening. She has two children, one in high school and one in college.

Jodi  Briggs

Jodi Briggs, RN, BS, Health Coach

Jodi Briggs has more than nine years of diverse nursing experience in cardiac care, progressive care, case management, orthopedic- neurology, and behavioral health. Prior to becoming a registered nurse (RN), she worked for more than 10 years in social services, mainly with the elderly population. She graduated with her RN from St. Elizabeth School of Nursing in Lafayette and received her BS from Purdue University. Previous positions include IU Health Arnett, IU Health and, most recently, Sycamore Springs in Lafayette.

Jodi has three children, ages 16, 13, and 6. She spends her free time with her family and attending the children's sporting events.

Debra Crume-Nagel, RN, BSN, Health Coach

Debra Crume-Nagel, RN, BSN, Health Coach

Debra Crume-Nagel has been in nursing for 25 years. Upon graduating from Ball State University with a BS in nursing, she worked for St. Vincent Hospital in Indianapolis for 15 years. She started at St. Vincent Carmel Hospital on the medical/surgical floor, and later transferred to the ICU department at St. Vincent Hospital on 86th St. in Indianapolis.

After moving closer to family in Carroll County, she was employed by St. Elizabeth Hospital in the CCU/PCU department. Finally landing in Rensselaer, Debra worked at Jasper County Hospital as a patient care coordinator, which involved moving throughout the hospital and assisting in all nursing departments.

Debra and her husband Duane have six children with ages ranging from 10 to 29. They enjoy volunteering at St. Augustine Church and spending time with their family.

Cheryl Laszynski

Cheryl A. Laszynski, RN, BSN, MBA, CHWC

Cheryl has 28 years of health care executive/management experience. She received a BS in nursing from Indiana University and an MBA from Indiana Wesleyan University. As the administrator/director of ancillary services for Arnett HealthSystems, she managed a variety of physician practices and clinical departments, such as urgent care centers, occupational medicine, pharmacy, lab, dietetics, and physical/occupational/speech rehabilitation.

Cheryl's clinical experiences include emergency/intensive care, home health care, managed care/benefit administration and quality improvement. Cheryl will look familiar to Purdue employees, as she has worked on campus for the past eight years. Most recently, she was the worklife administrator for WorkLife Programs. 

Cheryl is married and has two adult children. She enjoys running, cycling, reading and spending time with her family.

Suzanne Rossiter

Suzanne Rossiter, RN, BSN, MSN, Health Coach

Suzanne Rossiter has been a critical care registered nurse (RN) for eight years. She has worked at Jasper County Hospital in Rensselaer, Ind., and Methodist Hospital in Merrillville, Ind. Suzanne graduated in 2004 from Purdue University with a BS in nursing. She earned her master's in nursing education and graduated magna cum laude from Valparaiso University in 2011. Suzanne also has experience as a diabetes educator.

She and her husband have 3-year-old twin girls.

Jessica Green

Jessica Dicks, MA

Jessica Dicks has more than 10 years of administrative assistant/front office experience and three years of medical assistant (MA) experience. In 2010, Jessica received her associate's degree in applied science from Harrison College in Fort Wayne. Jessica previously worked at Unity Healthcare Internal Medicine in West Lafayette.

Jessica is married and has two daughters, ages 11 and 7.

Sammantha Klinger

Sammantha Klinger, MA

Sammantha Klinger graduated from Harrison College in October 2010. She received two associate’s degrees in applied science – one in medical assisting and the other in medical reimbursement technology. Sam has a total of six years of experience in both fields. She previously worked at Heartland Neurology and at Sue and Associates.

Sam resides in Lafayette and has two children, one daughter and one son. In her free time, she enjoys going to the gym, hiking, camping, traveling and spending time with family and friends.

Asha Ortega

 Asha Ortega, MA

Asha Ortega graduated from Harrison College. She has previous work experience in an orthodontics office, Westminster Village, WeCare TLC and the Subaru of Indiana Automotive clinic.

Asha has three children and resides in Monticello.

Monica Ortiz

Monica Ortiz, MA

Monica Ortiz graduated from Harrison College of Lafayette in 2012 with her associate's degree in applied science. Monica is certified through the American Association of Medical Assistants and has more than 10 years in customer service. She most recently worked at a dermatological practice in West Lafayette.

Monica resides in Lafayette, is married and has one daughter, 11, and two Boston terriers. In her free time, she enjoys camping; going to movies, concerts and Chicago Cubs games; and spending time with her family and friends.

Nicole Sparenberg, MA

Nicole Sparenberg, MA

Nicole Sparenberg graduated from Ivy Tech Community College. She has previous work experience in family practice and hospital settings and with WeCare TLC.

Nicole recently married and resides in West Lafayette. In her free time, she enjoys shopping, camping, boating, traveling and spending time with her family and friends.

Rosemary Benjamin

Kamal Singh, PharmD

Kamal Singh graduated from the Medical University of South Carolina in 2005 with her doctorate of pharmacy. In the past eight years, she has gained experience as a community pharmacist and pharmacy manager. She has worked for Eckerd Corp., Target Corp. and CVS during the course of her career in New Jersey, South Carolina and Alabama.

Kamal will be providing medication therapy management (MTM) services for Purdue benefits-eligible employees and their dependents covered on a Purdue medical plan.

Sara Primeau

Sara Primeau, ACSW, LCSW, EAP Counselor

Sara Primeau has more than 30 years of experience as a licensed clinical social worker. She graduated from Purdue University in 1973 with a BS in psychology and earned her master's of social work in 1979 from Indiana University. She is a member of several associations, including National Association of Social Workers, International Association of EAP in Education and the National Alliance of Mental Illness. She will be familiar to Purdue employees as she most recently worked as the EAP Counselor for the Purdue University WorkLife Employee Assistance Program. Previously, Sara worked at the Wabash Valley Alliance.

Sara and her husband of 34 years have three sons and two grandsons. In her free time, Sara enjoys gardening, reading and traveling.


Phlebotomist

Sylvia Castell, IOP

Sylvia Castell, IOP

Sylvia Castell has more than 20 years of experience as a phlebotomist and medical assistant. She has worked at Mid America Clinical Laboratories (MACL) as a phlebotomist for the past two years. Prior to that, she worked with Unity, Sigma Medical Group and IU Arnett Clinic. Sylvia also has previous work experience in occupational health, immediate care, family medicine, pediatrics and neurology.

Sylvia and her husband have two adult children – a daughter and a son – and three granddaughters. In her spare time, Sylvia enjoys card crafting, cooking and spending time with her family.


Practice Administrator

Millie Fenneman

Millie Fenneman, Practice Administrator

Millie Fenneman graduated from the University of South Florida with a BS in bio-medical science. In 2011, she earned her master’s of public health degree with a specialty in health policy and management. 

Millie began her health care career in 2010 when she was selected for a summer enrichment associate program at DaVita, Inc., a large dialysis organization. The program was offered through the American Hospital Association’s Institute of Diversity in Health Management. DaVita subsequently offered her an ongoing role within its organization as program manager for seven home hemodialysis and peritoneal dialysis clinics throughout central and southern Indiana. During her tenure with the company, she also led the education outreach program for chronic kidney disease prevention. Prior to DaVita, Millie held the position of strategic account manager for EmployBridge, Inc.

Millie and her husband Mitch reside in Lafayette with their 1½-year-old son, Jack. Mitch is a Purdue ABE alum and an avid fan of the basketball program. During basketball season, you may find Millie, Mitch and Jack cheering on Purdue from the stands. They also enjoy camping, boating, reading and spending time with their family and friends.


Administrative Assistants

Tammy Gentry

Tammy Gentry, AA

Tammy Gentry graduated from Indiana Business College (Harrison College) with an associate’s degree in office management. She has more than 15 years of experience in human resources, payroll and employee benefits. Most recently, she worked as a patient access specialist at a family medicine, dermatology and sports medicine office within the Franciscan Physicians Network. 

Tammy has four children and resides in Lafayette. She enjoys camping, bowling and spending time with family and friends.

Tina Hodson

Katina Hodson, AA

Katina “Tina” Hodson has more than 18 years of experience as a receptionist and assistant. Tina will look familiar to Purdue employees as she spent the past four years as the assistant for Purdue’s Employee Assistance and WorkLife Programs. Additionally, she previously worked as the receptionist for the Purdue Veterinary Clinic. Tina is also a pharmacy technician and has 10 years of experience in that role.

Tina loves reading, gardening and spending time with her family. She is married to her high school sweetheart. They have one daughter and one granddaughter. 

Maggie Rice

Maggie Rice, AA

Maggie Rice has 13 years of experience an administrative assistant and receptionist. She has worked in health care at various medical offices including the Danville Care Center, Dr. Conn Dental, and, most recently, Carle Physicians. Maggie specialized in computer programming at Danville Area Community College in Danville, Ill. 

Maggie recently moved to West Lafayette from Georgetown, Ill. She has two boys, ages 12 and 18. In her free time, Maggie enjoys hiking, canoeing and boating.
