Eligible Purdue University employees, retirees, and disabled staff now have the opportunity to enroll in a voluntary legal services plan.  Your enrollment covers your spouse or same-sex domestic partner and eligible dependents at no extra cost.

Many of you requested that we make this benefit available.  Following an extensive review process of the major providers of this benefit, Purdue selected the ARAG Group of Des Moines, Iowa as our provider for this coverage.  The Voluntary Benefits Advisory Committee, representing all staffs and all campuses, was extremely helpful in making this selection. 

ARAG has been offering legal plans for 70+ years.  The company represents approximately 500 employers and 12 million employees worldwide.  The ARAG benefit plan selected by Purdue is commonly referred to as UltimateAdvisor.  Please see the coverage overview for more details about the UltimateAdvisor program.

Cost of the Plan

The premium cost of the plan is $222.72 per year.  This rate applies regardless of whether you are actively employed, retired, or disabled. 

The materials you need for enrolling in UltimateAdvisor are mailed to you during open enrollment periods.  The next open enrollment period will be this fall.  To guard against adverse selection, those enrolling in the UltimateAdvisor plan during the Fall 2007 enrollment period will be required to remain in the plan until December 31, 2008.  

Your per-pay deduction amount for coverage will be:

      Biweekly, 26 pays per year = $ 8.57
      Biweekly, 18 pays per year = $12.37
      Monthly, 12 pays per year = $ 18.56
      Monthly, 10 pays per year = $ 22.27
 

How to Enroll

1)   If you wish to participate in the voluntary legal services benefit, please complete the enrollment form, which includes an authorization for Purdue to deduct your premiums from your pay.  If you are NOT eligible for payroll deductions (i.e., participants who are retired, disabled, or fellowship recipients), you must choose either direct billing or automatic monthly checking account deductions.   

• If you are electing the automatic checking account deduction, please complete and sign the Automatic Checking Account Deduction Authorization form, attach a voided check, and return it to USI Affinity with your enrollment form. 
   

• If you are electing the direct billing payment method, you will receive your 2008 annual billing statement on or about December 1, 2007.  You will be billed $222.72 for coverage from January 1 through December 31, 2008.

2)   Return the completed enrollment form to USI Affinity in the envelope provided in the enrollment packet.  

Important note:  If both you and your spouse or same-sex domestic partner work at Purdue in a benefits-eligible position, only one of you should enroll in the voluntary legal services plan since enrollment covers your spouse or same-sex domestic partner at no additional expense.
