Overview of the Tobacco-User Additional Premium

Employees certify their tobacco-user status and that of their covered spouses/SSDPs during the online benefits open enrollment process in the fall. Those covered by a Purdue medical plan who do not certify their status will be considered a tobacco user and will be charged the additional premium. The tobacco-user additional premium for 2014 is $500 per person.

To qualify as a non-tobacco user and avoid the additional premium, a person must have been a non-tobacco user for at least 12 months immediately prior to completing benefits enrollment. Employees and spouses/SSDPs who have used tobacco during the past 12 months may qualify for a waiver of the additional premium by completing an approved tobacco cessation program. Approved programs completed Sept. 1 through the end of open enrollment qualify for a waiver of the upcoming calendar year's additional premium.

Ways to qualify for a waiver after open enrollment

Complete an approved tobacco cessation program

If you complete an approved tobacco cessation program by one of the dates below, your additional premium will be waived on all pay you receive in the calendar year on or after the listed waiver date.

  • Complete approved cessation program by Feb. 28 and qualify for waiver of the additional premium beginning on April 1.
  • Complete approved cessation program by May 31 and qualify for waiver of the additional premium beginning on July 1.
  • Complete approved cessation program by Aug. 31 and qualify for waiver of the additional premium beginning on Oct. 1.

Reach 12 months as a non-tobacco user

Those who complete 12 months as a non-tobacco user during the calendar year can get the additional premium waived at that time.  When you have been a non-tobacco user for at least 12 months, complete the Tobacco User Certification Form and submit it to Tatiana Winship in Human Resources. Your additional premium will be waived on Purdue pay you receive during the rest of the calendar year, after allowing appropriate time for processing of your waiver.
