As a new employee, you are asked to make important benefit decisions for you and your family. Once your basic information is set in the Purdue system, the online enrollment system, Benefitfocus, will send an email to your Purdue email address asking you to log in and record your benefit elections. You are required to complete your enrollment within 30 days of receiving your Benefitfocus enrollment email.

Medical Coverage

Employees who do not enroll for medical coverage during their initial open enrollment period will have the following coverage:

  • Purdue Health Plan Plus HSA 2, employee-only coverage

  • No Purdue contributions to their health savings account (HSA)

  • $500 tobacco-user additional premium

The next opportunity to enroll in a Purdue medical plan will be during the annual benefits enrollment in the fall.

Flexible Spending Accounts

Disability Coverage

  • Short Term Disability - Offered to employees in a clerical or service staff benefit-eligible position. If you elect, enrollment opens during annual open enrollment and will begin with your one-year service anniversary.

Prepare to Enroll

During the enrollment process, you will be requested to submit required documentation for dependents you elect to cover. For information on eligible dependents and required documentation, click here. To prepare for the enrollment process, you may want to collect the following documents and/or pieces of information in advance:

  • Medical/Prescription
    • If adding dependents, you will need to submit required documents (i.e. birth certificates,    marriage certificates, etc.)
  • Life Insurance
    • You will be asked to enter beneficiary information, including names and birthdates.
  • Health Savings Account(HSAs) and retirement plans
    • Beneficiary information is requested and can be provided directly through your account login with PayFlex and Fidelity, as it is not a part of the required enrollment process.

Required documents may be uploaded electronically in Benefitfocus from the My Documents” menu option on the “Manage Your Benefits” page or submit a paper copy to Human Resources – Benefits in Kurz Technology Center, West Lafayette.

Begin Enrollment

To enroll, visit, select “Benefits Enrollment (Active) – Benefitfocus (Fac & Staff)” and log in with your Purdue career account.

Once logged in, you will see a personalized welcome page. To begin the enrollment process, click the “To Enroll, Make a Change, and View Your Benefits CLICK HERE” gold button located at the top right of your screen.

From the “Manage Your Benefits” page, select “Life Change” from the menu on the left and follow the steps to complete.

Once enrollment is complete and benefit elections are approved, benefit coverage and deductions will begin retroactively with your hire date. Insurance cards will be sent to your home address on file typically 10-15 business after you’ve completed your enrollment and submitted required documentation.

Helpful Tips

Should the need arise to use a benefit before you have received your benefit cards, here are some tips to help you:

  • If you are in a situation which requires up-front payment, such as filling a prescription, you will need to pay out of pocket and file a claim to your benefit vendor once your coverage becomes active.
  • If you are in a situation which involves a direct bill, such as a doctor’s visit, you can give your insurance information to your service provider when your coverage becomes active to have the claim(s) reprocessed.
  • In the case of an urgent situation, please contact the HR Service Center at (765) 494-2222 to see if we can expedite the enrollment process.

  • Note: You and each of your covered dependents will receive an insurance ID card from Anthem that will serve as both your medical and prescription insurance ID card.
  • Call the Purdue dedicated Anthem Customer Service at 855-502-6365.
  • Go to to find network providers, view the status of your claims, print a temporary ID card and much more.
  • Register an account with Castlight, the health care shopping tool, once you receive your insurance cards to compare in-network doctors and medical facilities based on the price you'll pay and quality of care.


  • Note: You and each of your covered dependents will receive an insurance ID card from Anthem that will serve as both your medical and prescription insurance ID card.
  • Call the Express Scripts member service line at 877-845-2795.
  • Register with Express Scripts for medication pricing information, home delivery, easy-order refills, and more.


  • To access your vision benefit, simply provide your 10-digit Purdue ID (PUID) to in-network vision providers, no insurance ID card needed.
  • Register with VSP to check your vision benefits and find a provider in VSP’s network.

Flexible Spending Accounts (FSA)/ Health Savings Account (HSA)

  • If you enroll in a Health Care FSA or HSA, PayFlex will mail one debit card to your home address.  To order additional cards for your dependents, call PayFlex at (800) 284-4885, or register with PayFlex using your 10 digit PUID as your Member ID without spaces or dashes.
  • With PayFlex, you can view your account balance, upload documentation, set up direct deposit and notifications for email or mobile, and more.
  • If you choose the Limited Purpose FSA, your HSA card will double as your Limited Purpose FSA card for dental and vision expenses.
  • There are no cards associated with a Dependent Care FSA.


  • If you enroll in a dental plan, you will receive an insurance ID card from Delta Dental. Alternatively, you may provide your PUID minus the first zero to an in-network PPO dental provider.
  • You may also register with Delta Dental to print a temporary ID card and view benefit eligibility and claims information in the Consumer Toolkit . 


  • Fidelity Investments, the retirement plan record-keeper, will send information to your home address.
  • Register your account with Fidelity to enter beneficiary information, update investment elections, view account balances, set up a voluntary savings account, and more.
  • Visit the Fidelity Investor Center located in the Purdue Memorial Union (PMU) for no-cost assistance with setting up your retirement plans and retirement planning.
