Eligible Dependents

Eligible dependents include:

(A)  Your legal spouse, so long as he or she is not covered as an employee under this plan.  When a person is no longer your legal spouse, that person no longer qualifies as your eligible dependent.

(B)  Your same-sex domestic partner (SSDP), so long as he or she is not covered as an employee under this plan.  When a person is no longer your same-sex domestic partner, that person no longer qualifies as your eligible dependent.

(C)  Each dependent child until that child reaches his or her 24th birthday.  The child must meet Internal Revenue Service qualifications for being claimed as a dependent on your taxes.  A child will not be covered if he or she is covered as a dependent of another employee or is married.

You will be asked to provide verification of you dependent 's eligibility if you enroll a dependent.

The term child includes any of the following who are dependent upon the employee for at least 50 percent of their financial support, until the child reaches the limiting age:

(A)  A natural child;

(B)  A step child;

(C)  A legally adopted child or a child legally placed for adoption;

(D)  A child under your legal guardianship; and

(E)  The child of your same-sex domestic partner who is a member of your household, or maintains your home as a primary residence while living away from home and enrolled as a full-time student.  To cover the child of your SSDP, you must cover your SSDP on your insurance.

Children who are physically or mentally incapable of self-support may continue on your Purdue coverage beyond the normal age limit if the disability continues and the child remains unmarried. The child must already be covered under the plan. You may be asked to provide certification of the child’s disability annually.

To find out what dependent coverage, if any, is offered for a particular benefit program, refer to the Web page that describes the benefit.
