Going on a Leave Without Pay?
What happens when you aren't being paid, so voluntary benefit premiums can't be deducted?
Each voluntary benefit is administered by the corresponding insurance carrier. When you go on a leave (or miss a pay for any reason), you need to make arrangements with the carrier to keep your premium payments up-to-date and your insurance in effect. Listed below is information about how each carrier handles premiums during a time when you're not being paid by Purdue.
Delta Dental of Indiana
Keeping Dental Premiums current
If you are paid biweekly and miss two deductions, or if you are paid monthly and are off work for more than 30 days, Delta Dental will transfer you to a direct billing arrangement, and you will receive a bill at your home address. The clock for being off work starts with your first missed deduction.
How to begin payroll deduction for Dental insurance again
Once you are able to have payroll deductions again, you will need to contact Delta Dental and ask to go back on payroll deduction.
Keeping Legal premiums current
If you attempt to access services during the leave period, Hyatt will contact Purdue to inquire about your eligibility. If you're eligible, Hyatt's support staff will manually add you back into Hyatt's system. You will need to work with the HR Service Center to ensure that your premiums are up-to-date.
How to begin payroll deduction for Legal insurance again
Once you are able to have payroll deductions again, you will need to contact Hyatt Legal and ask to go back on payroll deduction.
Universal Life/ LTC rider and/or Accident Insurance
Keeping Universal Life/LTC rider and/or Accident Insurance premiums current
If you have not had a payroll deduction for more than 60 days, Trustmark will transfer you to a direct billing arrangement, and you will receive a bill at your home address. Trustmark calculates your payments on a monthly basis. The 60-day clock starts with your first missed deduction. The insurance division will only send one bill, if you do not pay that bill, Trustmark will not send a second bill; however, the company will send statements. If you don't pay the premium bill, you will eventually be notified that your policy is about to lapse. If you pay the bill, a new bill automatically generates for the next month.
