Anthem to provide dental coverage in 2017

Nominate your provider if he or she is not currently part of the Anthem Dental Complete Network

Anthem is currently working hard to recruit additional dental providers to support the plan for Purdue faculty and staff. Several dentists have expressed interest in joining the Anthem Dental Complete network, and employees are asked to help the efforts by nominating dentists who are not currently participating in the Anthem Dental Complete network.

  1. First, check to see if a dentist is already participating in the Anthem network: Use Anthem’s detailed “How to Find a Dentist Online” guide.. In Step 3, employees will need to “Search as a Guest” since the Anthem dental account is not effective until Jan. 1. If a dentist is already participating, there is no need to nominate him or her.
  2. Second, if a dentist is not currently participating an employee can nominate a dentist to Anthem, and an Anthem representative will reach out to the provider. To nominate a provider, complete the Provider Nomination Form, then email or fax the form to Anthem as directed on the form.

Dental Insurance

Beginning January 1, 2017, the dental plan will move from Delta Dental to Anthem Dental. The dental plan will now be a separate election and no longer included in the medical plan. The separation of the dental benefit from the medical benefit will allow employees to select coverage for themselves or eligible dependents independently from the medical plan. The University will continue to provide preventive dental to employees and eligible dependents.

Benefits-eligible employees will have four options to choose from in EBenefits regarding dental coverage. 

  1. Preventive Only – Covers preventive services only. Different than last year, this will require an election in EBenefits separate from your medical plan election.  
  2. Anthem Dental Plan Option 1 (Point-of-Service) – This plan provides the broadest choice of dental providers.  While you will receive the greatest benefit for diagnostic, preventive and restorative work if the Anthem Dental Complete network is used, this plan allows you to use non-network dentists if you choose. Services provided by out-of-network dentists are covered at a reduced level and may be subject to balance billing.  Link to attached option 1 plan
  3. Anthem Dental Plan Option 2 (Standard) – This plan provides the most cost-effective benefits for diagnostic, preventive and basic treatments, but ONLY when an Anthem Dental Complete dentist provides the care. Very little coverage is provided when using non-network dentists.
  4. Opt out – You may choose to have no dental coverage, including preventive, by completely waiving all dental coverage.

How to Enroll (during annual open enrollment)

Dental plans are administered on a calendar year and are subject to the rules associated with Purdue's cafeteria plan. Your premiums are taken from your pay before taxes are calculated. Elections made during the annual open enrollment will be in effect from January through December, unless you have a qualifying change in family status.

Any deductible or plan limits associated with your plan are tracked on a January-December basis.

You may view and enroll in dental benefits using the online EBenefits system. Go to the OneCampus, choose EBenefits, and then sign on using your Purdue career account username and password.
