Tier 1 lab services at State Street Office Facility, West Lafayette campus
Tier 1 lab services are available at the Mid America Clinical Laboratories site at the State Street Office Facility (SSOF) near Purdue West. SSOF is located at 1601 W. State St., across from Horticulture Park. WorkLife Programs is also in SSOF.
Employees and dependents (including children of all ages) covered under any of Purdue’s three medical plans may walk in for service, or they may call 765-494-1872 for an appointment.
Hours
- Monday - Thursday, 7 a.m. - 5:30 p.m. Closed Fridays.
- Closed for lunch from 1 - 1:30 p.m. daily.
- Closed on University holidays.

- Fax: 765-474-8460. Physicians may fax lab orders to this number.
Using the lab
- You must have a written, signed lab order from your health care provider. Health care providers may fax your lab order to SSOF at 765-474-8460 or you may bring the order with you when you come to the lab.
- Please bring your Purdue medical plan ID card.
- Be sure to follow any special instructions from your health care provider (for example, fasting after midnight). Call the lab at 765-494-1872 for more information.
- All lab results will be reported directly to your health care provider. The SSOF lab does not receive your test results; they are available only from your provider.
- The SSOF lab is able to provide services only to Purdue faculty, staff and dependents covered by one of Purdue's medical plans.
