The Provider Choice is Up to You.
Purdue 500 is a PPO plan that allows you to use any medical provider you choose. Although this plan has a network, the Purdue 500 does not provide as much incentive for you to use network providers as the Incentive PPO plan does.
However, if you visit a network provider, you will be charged negotiated rates, and you will be protected from fees that are over the usual and customary (U&C) amount.
