Participating Labs for Incentive PPO and Purdue 500

Incentive PPO and Purdue 500 participants at the West Lafayette campus can use any of the labs listed here and receive a 100 percent benefit from the plan.  No deductible or coinsurance applies.   If lab work is drawn at your doctor's office, and the doctor sends the lab work to one of the labs on this list, the processing will be covered at 100%.

Providers may join or leave provider networks throughout the year.  Before having services, always check with the provider's business office to see if the provider is participating within the network.

Purdue University Student Health Center:

          PUSH -- Purdue Campus, West Lafayette

Report directly to the lab area in Room G04 on the ground floor of the north end of the Health Center.  Please DO NOT register at the main desk on the first floor. 

Please note: PUSH does not provide lab services for children under age 18, unless they are full-time Purdue students.
