Process

Use the Hotline to report your concern if you determine that alternate resolution channels will not work. Whether by phone or online reporting, you will be asked a series of questions in order to gather sufficient information for follow-up. You will be asked for contact information, however, you may remain anonymous if you choose. You may report financial matters, fraud, waste or abuse, health and safety issues, research noncompliance, violations of personnel or University policies, or other noncompliance.

Purdue is using a third party to facilitate reporting. Information is routed back through the Internal Audit Office so that appropriate administrators may handle the research and any associated recommendations or actions on the outcome.

At the conclusion of your contact, you will be supplied with a confirmation number so that you may check back in 14 days to see the status of your report. This feature also allows Purdue administrators to request additional information from you if you requested anonymity.

Please remember that the Hotline is not for emergency services. For emergency assistance, call 911.
