Graduate Application Information

PHD APPLICANTS, PLEASE CLICK HERE for application information and instructions.

Application information for students interested in MS-SLP and Au.D degrees:  Application Deadline is January 1st.  PLEASE READ ALL INSTRUCTIONS CAREFULLY.

Materials to be submitted to CDSCAS Centralized Application:

A. Online application (CSDCAS Application)

B. Application fee

C. Statement of purpose  (Guide to Writing a Statement of Purpose, Statement of Purpose Basics)

D. Three letters of recommendation (Recommendation Letters )

E. Official transcripts from all institutions attended

Materials to be submitted to Purdue University:

A. Online "Apply Yourself" application (Graduate Application)

i. Statement of Purpose - The Purdue application system will not let you proceed without completing this section, however, your statement of purpose should be sent to the CSDCAS centralized application service only. When completing this section, merely state "On file with CSDCAS", or submit any other statement along those same lines.

ii. Resume or curriculum vita.  This is an optional field in the application system, but is required by the department.

iii. List your recommenders.  Where it asks "Will this provider be submitting the letter of recommendation online?", click "NO".  Selecting NO as your answer will circumvent Purdue asking your recommenders directly for their letters. We do not want your recommenders to submit letters to Purdue.  Recommenders should submit letters to CSDCAS only.

iv. Complete the “Acknowledgements” section

B. Application fee

C. Official GRE scores (Purdue’s institution code is 1631; no department code needs to be specified)

D. Official transcripts from all institutions attended - Please submit a set to the SLHS Graduate Secretary at Purdue by February 15th. If you have courses in progress, please do not send those transcripts until after the fall grades have posted.  To be clear, you only need to send one set of transcripts to CDSCAS (by January 1st), and you only need to send one set of transcripts to Purdue (after any fall grades post, but before February 15th).  International transcripts must be sent to Purdue in their original form and in translation, along with copies of any diplomas awarded.  Please send transcripts to Purdue using the following address:

Purdue University
Speech, Language, & Hearing Sciences
Attn: Graduate Secretary
500 Oval Drive, Heavilon Hall
West Lafayette, IN 47907-2038

E. For those whose first-language is not English, we require official test scores less than 2 years old from one of the following: TOEFL iBT or IELTS.  Please note, the Department of Speech, Language, and Hearing Sciences (SLHS) has higher minimum requirements than what has been set by the Purdue University Graduate School.  Also, SLHS does not waive the requirement even if you have earned a degree within the past 24 months from an institution where English is the primary language of instruction.  English proficiency test scores are required of all international applicants whose first language is not English.

TOEFL iBT must have a minimum total score of 100 with minimum individual scores of speaking, 28; writing, 22; listening, 22; and reading, 22

IELTS must have a minimum score of 7.5 with no single band score lower than 7.0

If you are not sure whether or not you need to submit these test scores, please contact the Graduate Secretary.

Important note: Your application must be marked by CSDCAS as "Complete" no later than January 1st in order to be considered for admission.  It is ok if the application has not yet been verified or mailed. We must also have the Purdue "Apply Yourself" application by January 1st.

If you applying for a dual track degree, that is, you are interested in obtaining a Master's or Audiology degree and also a Doctoral degree, please follow the above CDSCAS instructions only for the MS or AUD degree and apply to the Purdue University Graduate School (Apply Yourself) as a doctoral applicant.  CLICK HERE for application information and instructions for completing the Purdue application for the PhD degree.

Please contact the graduate secretary (vpblack@purdue.edu) with any other questions.
