Internships

The Public Health and Applied Exercise & Health Major require students to complete an internship (HK 49200 Professional Work Experience). HK 26900 is a pre-requisite course to help students identify potential internship sites, and HK 49200 (1 credit) is the fulfillment of that internship. Please make certain to review thePracticum-Internship Manual Procedures (pdf) before beginning any internship program.

Students must complete the following steps prior to starting the internship experience as hours will not be eligible to count towards the internship without complete submission of materials:

1. On-site Practicum/Internship Job Description and Supervisor Resume

This is the most important step for the student prior to completing any paperwork for preparation of the internship. All internships must be approved by the Practicum Director, Lane Yahiro lyahiro@purdue.edu, to ensure it will meet the requirements for the course as established by the university. A job description is required. The job description must include: specific tasks, activities, or responsibilities the of the student intern; an identified timeline of the internship; and contact information for the agency supervisor. Once reviewed/approved by the Practicum Director, the student may complete the preparation materials for the internship noted below.

2. Liability Insurance

After a student has accepted an internship position, the student must apply for and pay fees ($13/year) to obtain Purdue University internship liability insurance. This can be done through the HK Business Office located in Lambert 101. This is the responsibility of the student.

3. Student Application Form

Students will be required to complete the Student Application Form (located in the Internship Manual) and submit it with a receipt of liability insurance to the Practicum Director. The Student Application Form does require a signature from the student, the agency preceptor, and the Practicum Director.

4. Registering for the Internship

Once students have completed all the arrangements for an internship as described above, they may register for HK 49200 (Professional Work Experience) with their academic advisor. Students must be signed into the course by the Practicum Director prior to registering for HK 49200.

This form MUST be submitted to the Internship Coordinator prior to the start of the internship. The form MUST be submitted electronically (i.e., via email).

All other internship coursework will be submitted via Blackboard after course registration.
