Change of Degree Objective (CODO) - Summer 2013
In order to CODO into the College of Health and Human Sciences, please follow these steps:
Step 1: |
Explore ALL pertinent information on the HHS CODO webpage including links to information on HHS unit pages. See Information Chart belowGeneral information related to majors
Information about CODO Requirements and Processes

Step 2: |
Once you have explored all of the pertinent information, complete the HHS CODO Interest Form. |
Step 3: |
Once you have submitted the HHS CODO Interest Form, call the contact that is listed for your major and make an appointment with an Academic Advisor.
NOTE: YOU MUST MEET WITH APPROPRIATE HHS UNIT ACADEMIC ADVISOR BEFORE BEING ELIGIBLE TO CODO, ADD A SECOND MAJOR OR DEGREE AND/OR IF YOU ARE AN HHS STUDENT WHO WISHES TO SWITCH TO ANOTHER HHS MAJOR IMPORTANT: Advisors will be available to meet with students who want to CODO into an HHS major, add a second major or degree, or HHS students who want to switch to another HHS major, from May 6- June 14, 2013 and July 8- August 16, 2013 (Advisors will not available from June 17-July 5, 2013 due to New Student Registration) |
Step 4: |
Download option sheet(s) for the HHS major(s) and enter your grades for the classes you have completed. |
Step 5: |
Bring your completed HHS option sheet(s) to your appointment with the appropriate HHS unit Academic Advisor. |
Step 6: |
If you meet the CODO requirements and are CODO’ing from a major OUTSIDE of HHS, then please make sure that you bring 2 copies of the Change of Curricula Form (http://www.purdue.edu/registrar/Students/CODO705_Form.pdf) to your appointment. You must have your current college fill out step one and sign the approval of your transfer.After you meet with your new advisor, and the Change of Curricula form is complete, return it to the Office of the Registrar in Hovde Hall, Room 45.**If you are switching majors WITHIN HHS, then you do not need the Change of Curricula Form. |
